Prior to your appointment
Welcome to Avon Chiropractic! Before you come in for your first visit, we would like for you to have certain paperwork filled out so we can get you in to see the doctor as quickly and as easily as possible. When you call to make your appointment, our receptionists will inform you which forms you need to complete. Located on the patient forms link are the new patient forms that you can print out at home and fax to us at (860) 673-7193. If you do not have access to a fax machine, mail or bring your paperwork to the office before your appointment. Please return your forms as soon as possible.
Your first visit to Avon Chiropractic Center will give you an opportunity to find out if chiropractic care can help you. You will receive a thorough case history and examination to make sure that your condition can be helped through the natural healing of chiropractic adjustments. As a doctor of chiropractic, Dr. Joanne Santiago believes in natural healing, including chiropractic treatments, rehabilitation, therapy, and nutrition.
Your first visit will consist of a consultation with Dr. Santiago in order to evaluate if you could benefit from chiropractic care. If we feel that chiropractic care will be beneficial, a complete health history examination and x-rays (if needed) will be done. Dr. Santiago will review your findings in great detail and on the following appointment date present to you your findings and proposed schedule of care. You will begin your chiropractic adjustments on that day and any other therapy treatments that your condition requires. However, if your condition requires immediate care, we will perform the necessary chiropractic treatment to start the healing process. Avon Chiropractic’s philosophy is to accommodate both chronic and acute need patients.
